Medication declaration for travel abroad

Are you going abroad and taking medicines covered by the Opium Act, such as strong painkillers? Then you will need a medicine certificate.

SCHENGEN application

You can use the Schengen declaration request the CAK. You can fill it in either digitally or by downloading the pdf, printing it and then filling it in. Then hand in the completed declaration to our practice. After a few working days you can collect the declaration, signed and stamped. Please note that the costs are not reimbursed by your health insurer. We charge €15.93 for this, payable by pin or cash.
