Do you live in postcode area 1431 BA – 1431 ZZ and are you not yet registered with a GP practice in Aalsmeer? Then you can register with the Westeinder GP practice. You can easily register via this registration form.

Practical information about your registration

• On your first visit, we ask you to bring valid identification (passport or ID card) and your health insurance details.
• By registering with us, you give us permission to deregister you from your previous GP. We also request that you inform your current GP yourself that you are switching.
• Please note that completing and submitting the registration form does not automatically mean that you are immediately registered. After we have received your form, we will contact you as soon as possible to let you know whether registration is possible.
