Patient satisfaction questionnaire

As a patient, you want to be sure that your GP practice provides good care. That is why our practice is tested and inspected every three years by the NPA. This is the accreditation institute of the Dutch College of General Practitioners (NHG).

To assess what is going well and what we can improve, the NPA has prepared patient questionnaires. Your opinion is very important here. By filling in the questionnaire, you help us to make our care as good and pleasant as possible for you. You can indicate in various sections how you experience our practice.

Completing the questionnaire will take about 5 to 10 minutes. Your answers will be processed completely anonymously.
